About the role?
Due to growth within the Home care packages team, we are seeking to recruit an experienced Case Manager ( Aged Care). Reporting to the Co-ordinator- client services and working within the Home care packages team, the incumbent will assist and support eligible recipients who have complex care needs and provide solutions and alternatives to residential care.

The incumbent will also promote engagement with all key stakeholders, and ensure homecare packages are planned, implemented and monitored in consultation with individuals, their carers, service providers and other significant people in the person’s life.

Requirements:

  • Tertiary qualifications in relevant health/ social sciences
  • Demonstrated commitment to the Purpose and Principles of annecto - the people  network;
  • Previous experience in case management in a community setting
  • Demonstrated ability to work efficiently with culturally diverse groups
  • Demonstrated ability to work autonomously with a high degree of initiative;
  • Excellent communication skills, both written and verbal.
  • Understanding and commitment to implementing Consumer Directed Care Initiatives;  
  • Current unrestricted Victorian Drivers licence.

  • Experience working in a similar capacity in a community based not-for-profit organisation;
  • Knowledge of community languages;

Selection Criteria Essential: Experience in supporting frail older people to continue residing in their home and connecting with their community
Essential: Knowledge of community services within the Northern Region
Essential: Understanding of the aged care reforms in relation to Home Care Packages
Essential: Experience in working within a person centred framework
Essential: Knowledge of a client data base.
